We need this in New Zealand to be able to manage rural shipping. We'd like the ability to have this add a cost line to the Checkout if you select a certain option.

E.g select "is this a rural address?" then add $6 delivery fee as a separate line to any shipping or other costs at checkout

A fancy (perhaps down the road version) might be able to trigger a different shipping rate.

Assuming this idea is similar to PPOM, I fully agree...
